Labs & Imaging:
Lab work and imaging provided by third-party services at discounted rates
Individual:
$90 monthly
Family:
$160 monthly for 2 family members
$60 monthly for each additional family member
$300 monthly maximum for household
Corporate:
Variable pricing based on needs
