Labs & Imaging:

Lab work and imaging provided by third-party services at discounted rates

10% discount for 6-month membership paid in advance

Individual:

$90/month

Family:

$160/month for 2 family members

$60/month for each additional family member

$300/month maximum for household

Corporate:

Variable pricing based on needs
