Cost based on complexity + materials used
Physicals
School, sports, or work physical: $60

Labs & Imaging
In-home lab tests: flu, urinalysis, glucose, etc...: $20-50
Other lab work and imaging fees paid directly to service providers at discounted cash rates

Payment due at time of visit (cash, card, HSA/FSA accepted)
No insurance billing

Individual: $80/month
Family: $125/month for 2 family members
$50/month for each additional family member
Corporate: Custom pricing based on group size and included services
Membership fees automatically charged on a monthly basis until cancelled
